Updated SMAP Forms and Education

The Ministry of Health will be changing mandatory documentation  requirements for the Saskatchewan Medication Assessment Program (SMAP) September 1, 2017.

To help pharmacists prepare for upcoming documentation changes and to provide additional support on the process of providing a medication assessment, PAS is providing a SMAP Educational Series, which will include:

  • a series of webinars (that will also be recorded) over the next 10 months,
  • a Pharmacist Guide to SMAP, and
  • live workshops in early 2018.

All education will be provided as a step wise approach to the medication assessment process.
